$99 enrollment fee per person
Ages 0-18 $40 mo. per child
Ages 18-29 #1 $65 mo. per person; #2 $60 mo. per person

65 + #1 $150 mo. per person; #2 #135 mo. per person
Family Membership
$250 enrollment fee per person
2 adults up to age 64, with child 0-26 living under same roof $350 per month (up to 4 kids, then an additional $40 per kid
