What plans do you offer?

Flexible Care

  Flexible care has no monthly fees—pay only when you need a visit.

  • $150 first-time visit; $90 for each additional in-person visit.

  • $50 telehealth visits (price may increase based on the number of concerns addressed).

  • Affordable urgent care visits for $50.

  • Optional procedures available for $100–$300, depending on complexity.


Subscription Care

Comprehensive, unlimited primary care with predictable monthly pricing, personalized based on age and family size for maximum value.

  • Unlimited in-person and telehealth visits included in the membership.

  • Discounts on procedures and urgent care services.

  • Family membership options for affordable, all-inclusive care.


Subscription Care Pricing

Individuals
  • $99 enrollment fee per person

  • Ages 0-18 $40 mo. per child

  • Ages 18-29 #1 $65 mo. per person; #2 $60 mo. per person

  • 30-64 #1 $125 mo. per person; #2 $110 mo. per person; #3 $95 mo. per person

  • 65 + #1 $150 mo. per person; #2 #135 mo. per person

Family Membership
  • $250 enrollment fee per person

  • 2 adults up to age 64, with child 0-26 living under same roof $350 per month (up to 4 kids, then an additional $40 per kid

  • Procedures $100-$300 based on procedure

  • $50 urgent care visits
